Molecular Specifications
Tesamorelin (brand name Egrifta) is a synthetic GHRH analog consisting of the full 44-amino acid GHRH sequence with a trans-3-hexenoic acid conjugate at the N-terminus. This modification provides resistance to DPP-IV cleavage at the critical Ala2 position — the primary site of native GHRH degradation — while maintaining full GHRH receptor binding activity. Tesamorelin carries the most extensive human pharmacological dataset of any research GHRH peptide, courtesy of its FDA-approved pharmaceutical indication.
GHRH Receptor Mechanism
Tesamorelin activates GHRH-R — a Gs-coupled GPCR on pituitary somatotrophs. Activation increases intracellular cAMP via adenylyl cyclase stimulation, activating PKA-dependent signaling that promotes GH gene transcription, GH synthesis, and pulsatile GH release. GH in turn stimulates hepatic IGF-1 production, completing the GH-IGF-1 axis.
